This course examines safe, quality, evidenced-based nursing documentation. Nursing documentation must have clarity, accuracy and accessibility for effective communication among health care workers. The role of organizational policies and procedures in guiding documentation is explored. Documentation practices concerning communication with the patient’s provider and provider orders, such as clarifying orders and receiving verbal orders are examined to support patient safety and patient outcomes.
